Health and Wellness Team
The Primary Health Care Mobile Health and Wellness Team travels throughout Colchester, East Hants, and Cumberland to provide Health and Wellness services in rural community areas. There will be an Emotional Wellness Counsellor/Navigator and a Family Practice Nurse on site to:
• Healthcare and social program navigation
• Access to programs, services, and community programs
• Health enhancement education and emotional wellness
strategies
Meet our Team:
Family Practice Nurse
Family practice nurses are registered nurses with specialized training in primary healthcare.
Emotional Wellness Counsellor/Navigator
Emotional Wellness Counsellors and Navigators are Social Workers or Registered Counsellors with an understanding of community resources and a background in coaching/counseling.
